Who We Serve

Serenity Path Network LLC partners with agencies, healthcare providers, veteran organizations, case managers, social workers, and community programs to provide structured housing and supportive services for adults working toward greater independence.

Additional programs for seniors, correctional re-entry, & women  populations will be announced as capacity expands.

Referral Eligibility

Agencies may submit referrals for adults who meet all of the following:

• Age 18 or older
• Able to participate in a structured housing program
• Willing to follow program guidelines and expectations
• Have a source of income, benefits, voucher, or funding plan or are actively working with an agency to establish one
• Do not require skilled nursing care, inpatient psychiatric treatment, or services beyond the scope of supportive housing

How to Submit a Referral

Step 1 – Join the Agency Waitlist

Complete the online Agency Referral form below in TEAL. Waitlist begins August 1, 2026.

Step 2 – Submit Referral Documentation

After referral form is submitted, any supported documentation that may include:

• Agency contact information
• Client demographic information
• Brief housing/service history
• Funding source or voucher information (if applicable)
• Current medications and healthcare supports (if applicable)
• Emergency contact information

Step 3 – Referral Review

Referrals are reviewed for program fit, funding eligibility, and bed availability.

Step 4 – Intake Scheduling

If accepted, Serenity Path Network LLC will coordinate an intake appointment with the referring agency and participant.

What Happens During Intake?

Intake appointments typically include:

• Program orientation
• Review of program agreement and expectations
• Collection of identification and required documents
• Discussion of goals, employment, benefits, and support needs
• Move-in planning and room assignment (when available)

Most intake appointments take 45–90 minutes.

Required Documents for Intake

Please help participants gather the following before intake:

• Government-issued photo ID
• Social Security card or verification
• Veteran documentation (DD214 or VA verification, if applicable)
• Insurance card (if applicable)
• Proof of income, benefits, or voucher authorization
• Current medication list (if applicable)
• Emergency contact information

Referral Priorities

Priority consideration may be given to:

• Individuals experiencing housing instability
• Individuals transitioning from shelters, treatment programs, or temporary placements
• Participants with active case management support
• Referrals with confirmed funding or voucher authorization

Communication Expectations

To ensure continuity of care, referring agencies should:

• Provide accurate contact information
• Respond to intake requests within 3 business days when possible
• Notify Serenity Path Network LLC of significant changes in the participant’s status prior to move-in
• Remain available for coordination during the first 30 days of placement when appropriate

Important Program Notice

• Serenity Path Network LLC provides supportive housing programming designed to promote stability, accountability, and independent living skills.
• Participation in the program does not create a traditional residential lease unless otherwise required by a funding source or housing assistance program.
• Referrals from agencies who are contracted recieve a discounted program rate.
